Enrollment / Health Update Forms

All families who previously attended the Whitnall School District should have received enrollment forms and information in May.  This information MUST be returned ASAP.  If you did not receive the forms, you may print out blank forms using the links above and bring them to the District Office located at 5000 South 116th Street, Greenfield, WI 53228.

New families to the district should stop by our District Office to complete a proof of residency.  This is required of ALL families attending the Whitnall School District and may be a lease or utility bill (driver's licenses are not acceptable).  Please feel free to print the enrollment and health form above and complete prior to stopping by as this paperwork will be required.  Once registration materials are accepted, you may visit the school(s) and pay school fees, visit with district administrators, or take a tour. In addition, parents of middle/high school students may consult guidance counselors to discuss scheduling. There will be follow-up “welcome back” days at each school building to distribute additional information and take school pictures.

Q: When is registration?
A: If you previously attended the Whitnall School District, your forms should have been mailed to you and these should be returned to the District Office ASAP.  If you are a new family, please stop by the District Office as soon as possible to complete paperwork.

Q: Where is the District Office?
A:  The District Office is located in Whitnall High School at 5000 South 116th Street, Greenfield, WI (
directions).  You may park along the tennis courts and enter the designated entrance.

Q:  I attended last year but I didn't receive any forms in the mail.
A:  Please stop by the District Office to pick up your forms (or download them using the links above and print them at home).  You should return the forms as soon as possible.

Q:  When are student pictures?  When do we pay other fees and take care of any additional items?
A:  Each school will be mailing additional information regarding registration and will hold a "back-to-school night" to follow-up on all items related to registration and returning to school.  Information regarding school pictures and several other items will be distributed at this time.  If you have questions regarding these items, please contact the school office.
